This form is to be completed by a department Chief, Training Officer, or designee of a Kentucky Fire Department (Career or Volunteer). Applications received directly from individuals wishing to attend the Recruit Class will not be considered. This is the official application for recruits based on your department's previous reservation for spots. We understand that departments may complete the hiring process just prior to a Recruit Class beginning. Please submit this application even if a recruit has not been officially hired. If the recruit does not complete the hiring process, we will remove the application. All of the requested information below is needed in a timely manner in order for us to prepare. One application may be used for multiple recruits (add recruit at bottom).
